DMCA and Copyright Policy
Effective Date: February 11, 2026 | Last Updated: October 7, 2026
1. Introduction and Purpose
QuikForms, LLC ("QuikForms," "we," "us," or "our") respects the intellectual property rights of others and expects its customers to do the same. This DMCA and Copyright Policy ("Policy") describes how QuikForms handles allegations of copyright infringement under the Digital Millennium Copyright Act of 1998, 17 U.S.C. § 512 ("DMCA"), and how copyright owners may report claimed infringement and how customers may respond with a counter-notification.
QuikForms is a managed package for Salesforce that customers ("Subscribers") install in their own Salesforce organization ("org") and use to build forms, surveys, and chat hand-off pages. Subscribers can put their own content into their forms, including custom HTML, logos, images, field labels, option text, and translated text.
QuikForms does not monitor, review, or pre-screen the content that Subscribers create or publish. QuikForms acts on notices of claimed infringement as described in this Policy.
2. Where Form Content Is Hosted
A QuikForms form is published on the Subscriber's own Salesforce Site or Experience Cloud site, and the form's content is stored in the Subscriber's own Salesforce org, which Salesforce, Inc. hosts. QuikForms, LLC does not host, store, or have access to that content, and it cannot edit or delete content in a Subscriber's org. The address of a form page is therefore on the Subscriber's domain or on a Salesforce domain, not on www.sfquikforms.com.
The material QuikForms itself hosts is limited to its own website at www.sfquikforms.com, including its documentation and its example plugins. The QuikForms AI gateway carries requests for AI features but does not store their content.
If you believe a form infringes your copyright, you may also contact the organization that operates the form, which is usually named on the form or on the website that links to it, and Salesforce, Inc., which publishes its own procedure for copyright complaints about content on its platform. Sending a notice to QuikForms does not notify either of them.
3. Where to Send a Notice
Send notifications of claimed infringement and counter-notifications to QuikForms by email:
Copyright Notices
Company: QuikForms, LLC
Email: info@sfquikforms.com
Subject Line: "DMCA Takedown Notice" or "DMCA Counter-Notification"
Notices sent to any other address, or without the subject line above, may take longer to reach the right person.
4. Filing a DMCA Takedown Notice
4.1 Requirements
If you are a copyright owner, or are authorized to act for one, and you believe that material QuikForms hosts or a form built with QuikForms infringes your copyright, you may send a written notification. Under 17 U.S.C. § 512(c)(3), the notification must include substantially the following:
- Identification of the copyrighted work. A description of the copyrighted work or works that you claim have been infringed. If one notification covers several works, a representative list is sufficient.
- Identification of the infringing material. A description of the material that you claim is infringing and that you want removed or disabled, with information reasonably sufficient to locate it, including:
- the URL of each form or page where the material appears;
- a description of the specific content element, such as a logo, an image, custom HTML, or field text; and
- any other information that would help locate the material.
- Contact information. Your name, mailing address, telephone number, and email address.
- Good faith statement. A statement that you have a good faith belief that use of the material in the manner complained of is not authorized by the copyright owner, its agent, or the law.
- Accuracy statement. A statement that the information in the notification is accurate and, under penalty of perjury, that you are authorized to act on behalf of the owner of an exclusive right that is allegedly infringed.
- Signature. A physical or electronic signature of the copyright owner or of a person authorized to act on the owner's behalf.
4.2 Incomplete Notices
If a notification does not substantially comply with 17 U.S.C. § 512(c)(3), QuikForms may not be able to act on it. QuikForms may ask the sender for the missing information but is not obliged to do so.
5. How QuikForms Responds to a Takedown Notice
5.1 Material QuikForms Hosts
For material on www.sfquikforms.com, QuikForms will act expeditiously to remove the material or disable access to it on receipt of a notification that substantially complies with Section 4.1.
5.2 Material in a Subscriber's Form
For material in a form, which is stored in the Subscriber's org, QuikForms will act expeditiously and within its technical means. Its response may include:
- sending the notification to the Subscriber's administrator contact and requiring the Subscriber to remove the material or disable access to it;
- suspending or terminating the Subscriber's QuikForms license if the Subscriber does not remove the material or disable access to it, or does not send a valid counter-notification, within the time QuikForms specifies; and
- taking any other action that is reasonably necessary.
5.3 Notice to the Subscriber
QuikForms will make reasonable efforts to notify the Subscriber promptly when it acts on a notification. The notice will include a copy or summary of the notification, information about the Subscriber's right to send a counter-notification under 17 U.S.C. § 512(g), and a reference to this Policy.
6. Counter-Notification Procedures
6.1 Right to Submit a Counter-Notification
If you are a Subscriber and you believe that material was removed or disabled because of a mistake or misidentification, you may send a written counter-notification to the address in Section 3 under 17 U.S.C. § 512(g)(3).
Important: Under 17 U.S.C. § 512(f), any person who knowingly and materially misrepresents that material was removed or disabled by mistake or misidentification may be liable for damages, including costs and attorneys' fees. You may wish to consult an attorney before sending a counter-notification.
6.2 Requirements
A valid counter-notification must include:
- Identification of the material. Identification of the material that was removed or disabled, and the location where it appeared before it was removed or disabled.
- Good faith statement. A statement under penalty of perjury that you have a good faith belief that the material was removed or disabled as a result of mistake or misidentification.
- Contact information. Your name, mailing address, telephone number, and email address.
- Consent to jurisdiction and service. A statement that you consent to the jurisdiction of the Federal District Court for the judicial district in which your address is located (or, if your address is outside the United States, any judicial district in which QuikForms may be found), and that you will accept service of process from the person who sent the original notification or that person's agent.
- Signature. Your physical or electronic signature.
6.3 QuikForms's Response to a Counter-Notification
On receipt of a counter-notification that substantially complies with Section 6.2, QuikForms will:
- promptly send a copy of it to the person who sent the original notification;
- inform that person that QuikForms will replace the removed material or cease disabling access to it in ten (10) business days; and
- replace the material or cease disabling access to it, which for a Subscriber's form means lifting any suspension QuikForms imposed because of that notification so that the Subscriber may restore the material, not less than ten (10) and not more than fourteen (14) business days after receiving the counter-notification, unless QuikForms first receives notice that the person who sent the original notification has filed an action seeking a court order to restrain the Subscriber from engaging in infringing activity relating to the material.
7. Repeat Infringer Policy
7.1 Policy Statement
In accordance with 17 U.S.C. § 512(i)(1)(A), QuikForms has adopted, and reasonably implements, a policy of terminating, in appropriate circumstances, the licenses of Subscribers who are repeat infringers.
7.2 Implementation
QuikForms will consider a Subscriber to be a repeat infringer if QuikForms receives three (3) or more valid notifications about material published by the same Subscriber within any twelve (12) month period, and for each of them the Subscriber either did not send a valid counter-notification or the person who sent the notification then filed a court action.
7.3 Consequences
When QuikForms determines that a Subscriber is a repeat infringer, QuikForms may:
- issue a formal warning to the Subscriber;
- suspend the Subscriber's QuikForms license;
- terminate the Subscriber's QuikForms license and subscription; or
- take any other action that QuikForms considers appropriate.
QuikForms may terminate a Subscriber's license for copyright infringement at any time, even without repeat infringement.
7.4 Record Keeping
QuikForms keeps records of the notifications and counter-notifications it receives and uses them to identify repeat infringers.
8. Content Types Subject to This Policy
This Policy applies to all content that Subscribers create, upload, or incorporate into forms built with QuikForms, including:
- Custom HTML content: HTML added to a form's head, header, or footer;
- Logos and images: image files used in forms;
- Field labels and descriptions: text entered as labels, descriptions, help text, or placeholder text;
- Select list options: option text for drop-down lists, radio buttons, and checkboxes;
- Custom HTML fields: HTML entered in Custom HTML fields;
- Translations: translated form text, whether written by the Subscriber or generated with QuikForms AI and published by the Subscriber; and
- Any other content: any other content a Subscriber incorporates into a form.
9. Misrepresentations and Bad Faith Claims
Under 17 U.S.C. § 512(f), any person who knowingly and materially misrepresents that material is infringing (in a notification), or that material was removed or disabled by mistake or misidentification (in a counter-notification), may be liable for damages, including costs and attorneys' fees.
QuikForms expects all parties to send notifications and counter-notifications in good faith, and may disregard submissions it reasonably believes were made in bad faith. This Policy is not legal advice. Anyone considering a notification or counter-notification may wish to consult an attorney.
10. Accommodation of Standard Technical Measures
In accordance with 17 U.S.C. § 512(i)(1)(B), QuikForms accommodates and does not interfere with standard technical measures used by copyright owners to identify or protect copyrighted works, where those measures meet the statutory requirements.
11. Limitations and Disclaimers
QuikForms does not monitor, review, or pre-screen Subscriber content and has no obligation to do so. Because forms and their content are stored in each Subscriber's Salesforce org, QuikForms cannot itself remove or alter that content, and its response is limited to the steps described in Section 5.2. By acting on a notification, QuikForms makes no determination that the material infringes any copyright, and by acting on a counter-notification it makes no determination that the material does not infringe. Nothing in this Policy limits QuikForms's rights or remedies under the DMCA, the Copyright Act, or any other applicable law.
12. Relationship to Other Agreements
This Policy supplements and is incorporated by reference into the QuikForms Terms of Service. If they conflict, this Policy governs DMCA and copyright infringement matters. The Terms of Service and the Acceptable Use Policy prohibit using QuikForms to infringe the intellectual property rights of any third party, and a violation may lead to termination independently of the repeat infringer policy in Section 7.
13. Modifications to This Policy
QuikForms may modify this Policy at any time. Changes take effect when the updated Policy is posted on this page with a new "Last Updated" date. Continued use of QuikForms after a change constitutes acceptance of it.
